Practice Facilitation
Facilitating Change, Facilitating Improvement
The facilitation work streams of the Clinical Effectiveness Group provides a proactive approach to assist NHS primary care organisations to meet targets and to enable them to understand their patients/populations. Working closely with practices we assist in the improvement of the data quality in the clinical record, reduction of inequalities and the development of clinical quality.
Through facilitation we can help with:
- Understanding read codes
- Using clinical system templates
- Understanding data sets - including dashboard reports
- EMIS web - searches and reports
- Secondary service information through QUTE
- Clinical Guidelines
We use a variety of methods including:
- In-practice group work
- One-to-one in practice sessions
- Groups of practices within clinical networks, consortia or polysystems
- Multidisciplinary workshops
- It training clinics
